Volunteer under 18

As you are under 18 years of age, parental/guardian consent is required to allow you to volunteer and for the storage of personal information. There are two options to apply:

Option #1 – Online

1. Download and have your parent/guardian complete the Parent/Guardian Consent form.
2. Complete the online Youth Volunteer Application form.

Option #2 – Mail / Fax / In person

1. Download the Volunteer Application form.
2. Print, complete and then drop off, mail or fax to: Concordia Hospital
